Inability to secure pick

I cannot keep the pick from moving out of my grip whenever I try to speed up when tremolo picking. It can occur while strumming and doing other forms of picking, but for simplicity, the tremolo displays the problem best and it seems to happen more quickly the faster I get. There have been times when I thought I fixed it and it wasn't an issue, but unfortunately I didn't know what I was suddenly doing right which means I have to figure out what I'm doing wrong if I'm going to get consistent. I'm trying a three finger grip because I can utilize the "tall-mouse" movement more readily for greater speed, but whether I use three or two fingers, it's the same issue.

I Had It and I Lost It

Months back, I actually modified my pick technique in such a way that I could pick really, really fast, both on one string and multiple. The attack was smooth and consistent and I no longer felt like I was fighting against the strings. It felt like I barely touched the strings in fact, so I kept this up for a couple weeks, and then life got in the way of things and I started learning to play songs again assuming that I had it “locked in”. Unfortunately, I didn’t and I seem to have relapsed to inefficient pick motions. There are bits and pieces of advice I’ve written down such as keeping the pick perpendicular to the string path, keeping the forearm from getting involved, keeping the wrist loose so it almost feels like I’m jiggling my hand when I pick really fast above 200 bpm. I believe there is probably something tiny and subtle in my movements that undermines my ability to pick efficiently and I'm trying to pinpoint the cause. I know the motto of this site is “do it fast first” but if I have bad habits then should I consider slowing it down and making sure the attack is smooth consistently before I speed back up? One improvement that I have retained is that the pick no longer rotates or pushes out of my grip so at least there's that
